PROGRAM DESCRIPTIONS


Assisted Living Facility

  • The Assisted Living Facility is for individuals who are HIV Positive and require assisted living, HIV positive individuals who are in Substance Abuse Treatment and homeless individuals discharged from a hospital or shelter with an acute condition (these individuals may or may not be HIV positive) who are in need of medical respite care.

Substance Abuse Treatment

  • Substance Abuse Treatment is an Intensive Day Treatment program and provides HIV positive individuals who have had treatment and relapse experience. They may live in the Assisted Living Facility and receive treatment 7 days a week with a progression toward independence. In addition, a Day Treatment program is provided to HIV positive individuals who have a safe living environment and support and need a less intense level of substance abuse treatment.

Mental Health Therapy

  • Mental Health Therapy serves HIV positive individuals who do not have an insurance source or who have Medicaid and are in need of mental health therapeutic services. Individual, family, couple, and group counseling is provided.

Case Management

  • Ryan White Case Management provides community based assessment, referral, linkage and follow-up for HIV positive Broward residents.
  • Medicaid PAC Waiver provides community and Home based services for individuals who have an AIDS Diagnosis, are disabled according to Social Security and meet financial limits for Medicaid Waiver.
  • ARTAS (Anti Retroviral Treatment Access Study) is a strength-based brief intervention for HIV positive individuals who are newly diagnosed or out of care for 6 months to obtain medical care.
  • AIDS Insurance Continuation Program (AICP) assists HIV positive individuals in paying private insurance premiums.
  • Ryan White II Medication Co-payment assists in paying co-payments for anti-retroviral and Prophylaxis medications for HIV positive individuals who do not exceed an income greater than 300% of federal poverty level.

Outreach and Education

  • HIV Testing and Counseling is provided at no cost and is strictly confidential. Can be provided on site or at your agency or organization.
  • Community Outreach and Education assists individuals in the community to connect with medical care and services.
  • SISTA Project is a program which assists African-American women on topics related to AIDS.

Intervention and Prevention

  • The MPowerment Project is a program serving outreach to gay men ranging in age 18 – 30 who seek education and support to prevent HIV infection.

Transportation

  • Transportation provides scheduled transport to medical appointments, pharmacy, support groups, food banks and other services based on eligibility.

Medical Services

  • For individuals living with HIV/AIDS, Broward House and the North Broward Hospital District provide on site medical services by a proficient team of physicians, registered nurses, pharmacists, nutritionists, lab technicians and case managers. (For appointment call 954-522-4749 ext 1237)
